Bupivacaine HCl
Indications/Uses
Spinal anaesth for surgery eg, urological surgery & lower limb surgery lasting 2-3 hr, abdominal surgery lasting 45-60 min.
Dosage/Direction for Use
Adult Urological surgery 1.5-3 mL. Lower limb & abdominal surgery (including hip surgery & caesarean section) 2-4 mL.
Contraindications
Hypersensitivity to amide-type local anaesth. Acute active CNS disease eg, meningitis, tumours, poliomyelitis, cranial haemorrhage; spinal stenosis & active disease eg, spondylitis, tumour or recent trauma eg, vertebral column fracture; septicaemia; pernicious anaemia w/ subacute combined spinal cord degeneration; pyogenic skin infection at or adjacent to puncture site; cardiogenic or hypovolaemic shock; coagulation disorders or ongoing anticoagulant treatment.
Special Precautions
2nd- or 3rd-degree AV block; neurological disorders. Intrathecal anaesth can lead to hypotension & bradycardia; reduce risk of such effects by eg, vasopressor inj. Inadvertent intravascular administration. High-dose use. Concomitant use w/ class III antiarrhythmics. May affect ability to drive & use machines. Severe hepatic & renal impairment. Reduce dose in late stages of pregnancy. Elderly.
Adverse Reactions
Nausea; hypotension, bradycardia. Post lumbar puncture headache; vomiting; urinary retention & incontinence.
Drug Interactions
Additive systemic toxic effects w/ other local anaesth or structurally-similar agents ie, class IB anti-arrhythmics. Class III anti-arrhythmics eg, amiodarone.
